IMPORTANT: if you are the head coach of more than one team participating in the event
we request that you fill out this form even if you included this information in your application:
Girls Coaching Conflict Form: Due Friday, Oct 21 at 7:00 PM
If you would like us to attempt to eliminate or minimize your coaching conflicts this form must be completed by the deadline.
